Poor medication knowledge.

  • Only about 19% of frail elderly patients are able to name their medications accurately.

  • Elderly people’s medications are supervised by others in at least 60%. 

  • Medication errors by older patients- forgetting to take them or double dosing are common in those with cognitive impairment/dementia.

  • If you can’t name your pills accurately including what they are for, potential side effects to watch out for and the doses in mg then you should have a pill pack loaded by the Pharmacist each week.
